Registration for all Athletes…

Congratulations on having been selected to represent the Carson Graham Eagles!  

There are 4 key steps that need to be taken to complete your registration for the upcoming season.  

Follow these steps, print the appropriate pages and return them to your Athletic Director or your Coach.  You are ineligible to compete in any sport until you have submitted every form completed and signed by a parent or guardian. 

This process should take approximately 10 minutes to complete. Please follow the instructions below:

1. Please read the Athletics Standards Form. This form outlines the standards and expectations for every student-athlete participating in a sport at Carson Graham AthleticStandardsForm.pdf
2. Complete the Student medical form.pdf, Print the document, and return it to the Athletic Director/Coach
3. Complete the Carson Athletics Informed Consent.pdf. Print the form and return it to the Athletic Director/Coach. Please refer to the full sport-specific informed consent package provided to your student at the team meeting at the start of the season prior to signing this form.
4. Athletic fees must be paid via School Cash Online Students must pay their fees before they receive a uniform to play in their first regular season game. Please contact the Athletic Director if you require assistance with fees.
